  General Release
  I, the aforenamed parent, grant permission for the aforenamed child(ren) to participate in activities with Saint Joseph Youth and I warrant that my child(ren) is in good health. In consideration of my child’s participation, I agree to hold Church of Saint Joseph and Archdiocese harmless from any and all claims resulting in my child(ren)’s participation in this event. I further agree to indemnify Church of St. Joseph and the Archdiocese of St. Paul & Minneapolis from any claims or lawsuits brought against the Church of Saint Joseph/Archdiocese of St. Paul & Minneapolis by myself, my child(ren) or others, that arises out of any behavior by my child(ren) at any Saint Joseph Youth event/activity and for any harm my child(ren) incurs by reason of their participation in the event. I also agree to pay reasonable attorney’s fees or expenses incurred by the Church of St. Joseph and the Archdiocese in defense of such a claim/suit.
  
  I agree that I am responsible for my child(ren)’s conduct and actions. The event sponsor is not responsible for any injury or damage incurred or caused by my child(ren). I understand that my child(ren) is required to comply with the Code of Conduct provided by the parish while participating in the event. I understand that if my child(ren) violates the Code of Conduct he/she may be required to be transported home at my expense.
  
  In the event of an emergency, I give permission to transport my child(ren) to a hospital for medical treatment. I agree to allow my child to receive emergency medical treatment at my expense at the discretion of the event sponsor. I wish to be advised prior to any further treatment by a doctor or hospital.

  Code of Conduct
  All participants are expected to follow the rules below while participating in Saint Joseph Youth programs. Please review them with your child:
  --Treat all other persons with respect.
  --Not cause any intentional harm (physically, emotionally, or spiritually) to any person in any way.
  --Respect the property of others, including all program facilities and property.
  --Follow all appropriate instructions of all personnel (staff, chaperones, youth directors, transportation personnel, etc.)
  --Be on time for all activity check-ins and departure time.
  --Not have in my possession any tobacco, alcohol, controlled illegal substances, or weapons.
  
  Modesty Guidelines
  --Shorts must be at least a five inch inseam or past the finger tips
  --Tank tops must be at least 2 fingers in width (no spaghetti straps or muscle tanks).
  --No biker shorts except for under skirts/dresses.
  --No midriffs showing.
  --Necklines may go no lower than four fingers below the collar bone.
  --No "lamp-shading" (shirts covering shorts completely).
  --Leggings must be covered by a long shirt/sweatshirt all the way around.
  
  Technology
  While on retreat, phones are not allowed. Leader contact information will be given to parents in case of emergency.

  The total cost for retreat is $225 per person. Please pay anywhere between the $50 non-refundable deposit up to the total cost of all your children attending. (Individual forms are needed for each child, but you may pay for your whole family on one form.)
